where can I translate the countries from the country comboboxes?

You can use Joom!Fish for that. If you installed nBill first, and Joom!Fish afterwards, you will need to copy some files over as explained here: http://www.nbill.co.uk/documentation/index.php?page=addjoomfish.html

That doesn't work in my installation. I have tried Joomfish 1.8.2 first, but it gives me

Warning: session_start() [function.session-start]: Cannot send session cookie - headers already sent by (output started...
when I try to "apply" for example a translation for a country. Joomfish 1.7. gives me SQL errors when I want to translate some nbill stuff.

I installed Joomfish after nbill and followed the install docs. Translations for other components, content or menus work fine.

Although I have not been able to re-create the problems you mention, it does look like the country code translation files are not working properly. I will address this for the next release.

The problem with translating countries in nBill is down to a limitation in Joom!Fish (it will only allow you to enter a translation if the primary key of the table is numeric). I have reported this and a proposed fix (which you can implement by hacking your own copy of Joom!Fish if you wish), as detailed here: http://www.joomfish.net/forum/viewtopic.php?f=21&t=376
